A New Resource for Parents, Family Members and Caregivers of Youth with Disabilities or Chronic Health Conditions

Make a New Year’s resolution to plan for a fulfilling and independent life as an adult by taking your seat at the Transition AHEAD Roundtable!  

Youth with disabilities or chronic health conditions and their parents or caregivers are invited to a day-long experience to set goals for a successful transition to adulthood.

Together with a team of peer mentors, family advocates, adult service providers and transition professionals, you will get answers to questions like…

What are my strengths? How can I turn my passion into a career? What does living independently look like? What tools do I need to advocate for myself? 

WHAT DO I GET OUT OF IT?

PERSONALIZED TRANSITION ACTION PLAN  You get an action plan for your child based on data and information collected through interviews, assessments, discussions, and activities conducted during the Roundtable. You and your child drive the process.         You will meet one-on-one with representatives from community organizations like Raising Special Kids, Direct Advocacy & Resource Center, and Ability 360 who share information with you about their services.

 A TEAM OF EXPERTS -- You will have access to a team of experts with years of disability experience from the University of Arizona Sonoran Center for Excellence in Disabilities.       PIECE OF MIND -- You and your child leave confident knowing you have a plan and the tools necessary for your child to transition to adulthood successfully.
