Some discussions are harder than others.  Talking about our wishes, hopes, and plans are an important and necessary conversation to have when we face a serious illness and the end stage of our life.  It is never too early to start this conversation among individuals with disabilities, families, health care providers, and people who provide support. The “My Life, My Wishes: Sharing My Journey” is a workbook that helps people write about things that are important to them, identifies the decisions that need to be made, and includes health care options to discuss with health care providers.
