Arizona Statewide Independent Living Council (SILC) Healthcare Experience Survey

Have you experienced difficulties accessing healthcare services? Have you ever been told that a test or exam could not be done because the office or equipment was not accessible because of your disability? Have you had difficulty communicating with a doctor because an interpreter or assistive listening device was not provided to you? Is filling out forms difficult because your medical provider fails to make them available in an alternative format or does not offer assistance completing them?

The Arizona Statewide Independent Living Council has launched a survey designed to gather input from people who have disabilities, their family members and caregivers from across Arizona. The survey seeks to gather information in areas including physical access to healthcare facilities, issues faced by individuals with sensory related disabilities, how medical providers respond to accommodation requests and provide individuals with a platform for sharing healthcare related experiences.

The survey should take approximately 10 minutes for you to fill out and submit. If you require assistance or need an accommodation such as a copy of the survey in an alternative format, please contact the SILC office at 602-262-2900.
