The goals of the Stars-Champions for Change is increase consumption of fresh healthy food and promote physical activity for adults with developmental disabilities for the purpose of reducing health disparities related to chronic disease in this population.  The project is integrated into the services provided at the medical home clinic located at the Family and Community Medicine Clinic, UAMC South community kitchen and the Kino YMCA, the Stars Program takes cohorts of young adults with developmental disabilities between the ages of 18-24 to participate in a 12-week program to learn good nutrition basics and how to be more active.  The project adapts and implement evidence based interventions for this particular population; the project provides accessible information and strategies to improve health.
