Aging and Transitions Project
People with disabilities in Arizona comprise a population that is growing, aging, and becoming increasingly diverse. This statewide project has three major objectives: describe and address barriers to effective late life transitions for aging caregivers of adults with developmental disabilities (DD); promote effective late life transitions for adults with DD; and improve aging-related and end-of-life care for people with DD. Data and input will be collected from aging caregivers/family members, people with DD, service providers, advocacy groups and agencies, and health care providers. The outcomes will be an increased understanding of the needs of aging caregivers and people with DD and the strategies they utilize to plan for their future; the development of appropriate educational materials/training for service providers and families; and the promotion of effective transitional planning policies and end of life care for individuals with DD in Arizona.

MEDICAL HOME FOR YOUTH AND ADULTS WITH DEVELOPMENTAL DISABILITIES
The Sonoran UCEDD is creating a patient centered Medical Home for adults with developmental disabilities at its UPH Kino Clinic. The Medical Home has a physician led team with social worker support to provide the patient with coordinated and comprehensive care.

HEALTH EDUCATION PROJECT
Health Promotion for Individuals with Developmental Disabilities
The Sonoran UCEDD is currently developing an online information center to serve as a portal to useful web-based materials promoting the health and wellness of individuals with developmental disabilities. This center will also feature a series of fact sheets on health and wellness topics developed by UCEDD faculty and staff and structured utilizing complementary tracks: one track with an emphasis on readability designed for individuals with developmental disabilities, a second track designed for readers with a higher literacy level and targeting individuals with developmental disabilities and caregivers.

Educating Health Professionals
The Sonoran UCEDD is working to improve the education of health professionals to better prepare them to work with people who have disabilities. We have developed and are teaching a required curriculum to second year medical students on development. We have established an interprofessional conference on disabilities for students from the entire health science campus. We are teaching family medicine residents about caring for people with disabilities, with an emphasis on disability culture and history.


EMPLOYMENT PROJECT
The Sonoran UCEDD is serving as a consultant to the Arizona Governors Council on Developmental Disabilities (GCDD) in the development of a statewide conference on promising practices in employing individuals with disabilities. The AZ GCDD as part of its 5 year strategic plan has identified a need for training for employers to improve access to work across the state. The GCDD will provide monetary grants in 2009 to businesses in Arizona who apply with innovative ideas to increase the employment of individuals with disabilities at their businesses. In order that businesses across the state are well positioned to apply for these competitive grants, the GCDD and the UCEDD are developing a statewide conference to assist businesses in: universally designing the workplace; understanding how recruiting and retaining individuals with disabilities can improve the business' bottom line; understanding technology and other workplace accommodations.

BORDER INITIATIVE AND CONFERENCE
The Sonoran UCEDD working with community partners on both sides of the border is planning a bi-national, hands-on conference on disability as well working to develop sustainable projects in areas of need.

SOUTHERN ARIZONA MODEL PERSON CENTERED PLANNING PROGRAM
Person Centered Planning (PCP) Model Program for youth with developmental disabilities (DD) in transition to adult status living in Pima, Pinal and Cochise County, Arizona. This model will train families and consumers as well as service professionals in using “Essential Lifestyle Planning” (ELP) to plan and mobilize services to meet their needs and will build on the strength of community stakeholders in education and employment to develop a curriculum for adult living. In addition, the community stakeholders will be trained using “Framework for Accomplishments” to develop a culture of community involvement and support.



Interprofessional Education Day on Disability co-sponsored by the Sonoran UCEDD in November 2007 brought together nursing, medicine, pharmacy, law, public health and social work students to work on disability related case problem solving.
