What is a Person-Centered Planning (PCP) Facilitator?
An individual who has been specially trained to guide persons with disabilities through the PCP process.
What is Person-Centered Planning?
A positive and strength-based process that assists individuals with developmental disabilities to identify the steps needed to make their dreams become a reality.
What is the purpose of this training?
To recruit facilitators for the Person-Centered Planning Model Program. Your participation is free; however, upon completion of the training, you will be asked to complete at least one plan for the program.
What is the Person-Centered Planning Model Program?
The Person-Centered Planning Model Program is a three-year, federally-funded grant project co-managed by the Arizona Department of Economic Security/Division of Developmental Disabilities (DES/DDD) and the Sonoran University Center of Excellence in Developmental Disabilities (UCEDD). This grant focuses on young adults with developmental disabilities age 16-22 who are transitioning into adulthood. Facilitators play a vital role in the grant and will work with these young adults with developmental disabilities to lead and/or actively participate in their own PCP process.
What can I expect from this training?
You will gain the skills and knowledge needed to facilitate a person-centered plan
You will be able to apply the concepts of Person-Centered Thinking and Planning to all areas of your life – work, home, community
You will have the opportunity to enrich the lives of young adults with developmental disabilities and their families
