Are you a youth with a disability** age 16-22 and looking for a way to explore your hopes and dreams for your future? Do you want to connect with other youth who are also beginning to plan for their transition into adulthood? If so, then you should attend this session on Group Person-Centered Planning.

What is Group Person-Centered Planning?
Group Person-Centered Planning is a special opportunity for a group of people with disabilities, their family, friends, and others to come together and create Person-Centered Plans. Person-Centered Planning is a positive process that can help you to identify the steps needed to make the dreams of your future come true. You will be the center of your plan and it will be built on your interests, gifts and strengths.
What can I expect from the Group Person-Centered Planning session?
- The event will be attended by 5-6 other people with disabilities and their “circles of support.” A “circle of support” includes any family member, friend, service provider or other person that plays an important role in your life. Members of this circle must be invited by you in order to participate
- You and your circles will be able to share your ideas and plans with the other participants. By working as a group, many more ideas are shared and community connections are made
- Each group will leave with a poster which says all of the work that has been done and with a few action steps to get started
- About 2 months after we first meet, the whole group will get back together to discuss their plans and talk about other ways to move the plan forward
- The group should expect to get together at least 2 more times over the next year to reconnect and support each other to complete their individual plans
Registration is required for this event and attendees must attend an orientation session in order to participate (date to be determined). The deadline for RSVP’s is November 16th, 2009.
